Case Study/ Case Conceptualization Report
The case analysis is an assignment for assessing your ability to communicate client information effectively, identify the factors that promote or limit client functioning, evaluating ethical and legal considerations, service/treatment planning, and conceptualizing the case using counseling and developmental theories. This report should be typed, double spaced, and 6-8 pages in length. This paper should be professionally written and proofread carefully. Although this case study should be completed on a real person please take all steps necessary to protect the identity and confidentiality of the individual.
I. Case History
The first section of the paper introduces the client’s information, presenting problem, and social history.
1. Background and Identifying Information
The first section of your paper will present your clients age, sex, ethnic background, socioeconomic status, referral source(s) and any other demographic information that you feel is applicable.
2. Description of the Presenting Problem
In the next section of your case study, you will describe discuss the chief complains, significant emotions in the clients words and according to your observations. Discuss the major client issue(s) in this section.
3. Social History of the client
In this section discuss the family history, current family and peer relationships, marital history, developmental history, employment history, current living situation, medical history, psychiatric history, counseling/service history, drug/alcohol history, and suicide/homicide history. Based on the strengths approach also discuss client strengths, successes, and resources.
4. Diagnostic Impressions of the client
Provide your diagnostic impression of the client using the DSM’s 5 narrative format. This information may come from the clients chart or if not available your impressions formed from your training in the Abnormal Psychology class. Explain how you reached your diagnosis (weather from the chart of your diagnosis), how the clients symptoms fit the diagnostic criteria for the disorder(s) or any possible difficulties in reaching a diagnosis.
II. Intervention
The second section of your paper will focus on the intervention used to help the client.
1. Ethical and Legal Considerations
Discuss in detail the legal and ethical considerations related to the client’s case and treatment.
2. Describe service/treatment Goals
Based on the information above what are the client goals based on client needs, diagnosis, and resources available. What type of treatment/helping relationship would you use with this client? How often would services occur? Identify anticipated response, any recommendations and your assessment of the client’s prognosis in treatment.
III. Theoretical Applications
This section of the paper requires you to analyze the client’s case from 4 different theoretical perspectives.
1. Humanistic Approach
Describe a specific humanistic approach that could be used to treat your client. Explain how a humanistic therapist would approach treatment. Provide information on the type of treatment you chose and the specific interview and counseling skills that would be used. Explain why the treatment should be successful or unsuccessful.
2. Cognitive Approach
Describe a specific cognitive approach that could be used to treat your client. Explain how a cognitive therapist would approach treatment. Provide information on the type of treatment you chose and the specific interview and counseling skills that would be used. Explain why the treatment should be successful or any difficulties that may result from this theories use.
3. Behavioral Approach
Describe a specific behavioral approach that could be used to treat your client. Explain how a behavioral therapist would approach treatment. Provide information on the type of treatment you chose and the specific interview and counseling skills that would be used. Explain why the treatment should be successful or any difficulties that may result from this theories use.
4. Developmental Approach
Describe a specific human development theory that could be used to understand your client. Explain how the theory could be applied in the client’s case and what impact it would have on treatment.
